NPIMap only uses public data published by U.S. government agencies and standards bodies. Below is every dataset currently on the site, what it contains, how we use it, its known limitations and the exact file loaded.
NPPES NPI Registry — Downloadable File
The National Provider Identifier (NPI) is the unique 10-digit identifier that HIPAA requires for health care providers in standard transactions. NPPES assigns NPIs and publishes the data providers submit: legal names, practice and mailing addresses, phone numbers, health care provider taxonomy (specialty) codes with state license numbers, other identifiers such as Medicaid numbers, secondary practice locations, electronic endpoints and, for organizations, the authorized official.
How NPIMap uses it
- Every NPI page: identity, entity type, names, addresses, phone numbers and enumeration dates
- Specialties and state license numbers, other identifiers, other names and endpoints
- Deactivated and replaced NPIs (served as "gone" or redirected to the replacement NPI)
- Browse pages by state, city and specialty (primary taxonomy and practice location)
Limitations
- Information is reported by providers and is not verified by CMS.
- Providers often do not update their records; the "last updated" date is shown on every page.
- NPPES does not publish relationships between NPIs, and Employer Identification Numbers are withheld.

Medicare Fee-For-Service Public Provider Enrollment
Enrollment records for providers and suppliers approved to bill Medicare fee-for-service: NPI, PECOS associate control ID (PAC ID), enrollment ID, enrollment state and provider type. Companion extracts list benefit reassignments (individuals who reassign their right to Medicare payment to a group or organization), additional NPIs on an enrollment, practice location cities and secondary specialties.
How NPIMap uses it
- Medicare enrollment details on provider pages (enrollment IDs, PAC IDs, states, provider types, specialties)
- Organizations & group practices: an individual who reassigns Medicare benefits to an organization
- Practitioners who reassign benefits to an organization, shown on the organization’s page
- Other NPIs on the same enrollment or with the same PAC ID
Limitations
- Covers Medicare fee-for-service enrollment only, not Medicare Advantage or Medicaid.
- A reassignment shows that an organization bills Medicare for an individual’s services. It usually reflects employment or a contract, but the file does not say which.
- The extract files can be older than the main enrollment file; each file’s date is listed below.

Medicare Facility Enrollments, All Owners and Changes of Ownership
Enrollment details for hospitals, skilled nursing facilities, home health agencies, hospices, federally qualified health centers and rural health clinics: legal and doing-business-as names, CMS Certification Number (CCN), NPIs, incorporation, organization structure, proprietary or non-profit status and practice addresses. The All Owners files list the owners and managing parties disclosed on each enrollment: organizations and individuals with 5% or greater direct or indirect ownership, operational or managerial control, corporate officers and directors, and managing employees, with ownership percentages, association dates and owner types such as private equity company, REIT or holding company. Change-of-ownership files record acquisitions, mergers and changes of ownership between enrollments.
How NPIMap uses it
- Medicare certification (CCN) details on facility pages
- Owners & managing parties of a facility, and the facilities an owner controls
- Facilities that share an owner, and change-of-ownership history
Limitations
- Owners are identified by PECOS associate ID. NPIMap links an owner to an NPI page only when that associate ID belongs to a Medicare-enrolled provider; other owners are listed by name.
- Change-of-ownership files in the loaded data cover home health agencies, hospices, FQHCs and RHCs.
- Ownership percentages are shown as reported; many roles (for example officers and managing employees) have no percentage.

Provider of Services Files
Characteristics of facilities certified to participate in Medicare and Medicaid, by CMS Certification Number: facility type, certification and original participation dates, termination status, bed counts, ownership or control type, and related or parent provider numbers. Published from the QIES and iQIES survey systems, with a separate file for CLIA-certified clinical laboratories.
How NPIMap uses it
- Certification status, beds, facility type and control type on facility pages
- Related certifications: parent providers, hospital units and cross-referenced provider numbers
Limitations
- Provider of Services files do not include NPIs. NPIMap connects them to NPI pages through the CCN on facility enrollments, so facilities without a matching enrollment are not shown.
- Clinical laboratory (CLIA) records have no NPI or CCN; NPIMap connects them to NPI pages only by name and address matching, labeled as such (see the methodology).

Opt Out Affidavits
Physicians and practitioners who have filed an affidavit opting out of Medicare, so they contract privately with Medicare beneficiaries instead of billing Medicare, with the opt-out effective and end dates.

National Provider Directory (NPD)
CMS’s directory of health care providers and organizations, published as FHIR resources that follow the HL7 National Directory of Healthcare Providers & Services (NDH) implementation guide: practitioners with state licenses, credentials and CMS verification flags; organizations grouped by tax identifier; practitioner roles that connect clinicians to organizations and practice locations; organization affiliations; insurance networks and plans; and electronic endpoints such as Direct addresses and FHIR APIs.
How NPIMap uses it
- Practice roles (current and past) between clinicians and organizations
- Practice locations, state licenses, credentials, languages and endpoints
- Organizations that share a tax identifier, and member-of affiliations between organizations
- Insurance networks and Medicare Advantage plans a provider participates in
Limitations
- Organizations are grouped by a hashed tax identifier; NPD does not publish the tax ID itself.
- Inactive practitioner roles are shown as past affiliations because NPD does not publish end dates.
- Some tax-ID group names are missing or copied from a single member; NPIMap then uses the most common member name.
- The HHS exclusion list flag is shown as reported by NPD and should be confirmed with the OIG.

Medicare Care Compare Provider Data
The datasets behind Medicare Care Compare: the Doctors and Clinicians national file (education, specialties, group practices, practice addresses, telehealth and Medicare assignment), clinician facility affiliations, procedure utilization and Merit-based Incentive Payment System (MIPS) scores; general information and star ratings for hospitals, nursing homes, home health agencies, hospices, dialysis facilities, inpatient rehabilitation facilities, long-term care hospitals and ambulatory surgical centers; and the Medicare supplier directory for medical equipment and supplies.
How NPIMap uses it
- Group practice membership and hospital or facility affiliations
- Clinician education, specialties, telehealth, Medicare assignment, MIPS scores and procedure volumes
- Star ratings and facility details on hospital, nursing home, home health, hospice, dialysis and other facility pages
Limitations
- Covers clinicians and facilities that participate in Medicare.
- Facility files are keyed by CCN; NPIMap shows them on NPI pages through enrollment files that list both identifiers (ambulatory surgical centers publish their NPI directly).
- Star ratings summarize specific measures over specific periods; see Medicare Care Compare for the methodology.
- Procedure counts are published as ranges.
- Dialysis facilities and medical equipment suppliers are published without NPIs; NPIMap connects them to NPI pages by name and address matching, labeled as such.

List of Excluded Individuals/Entities (LEIE)
Individuals and entities that OIG has excluded from participating in Medicare, Medicaid and all other Federal health care programs, with the exclusion authority, exclusion date and any reinstatement or waiver.
How NPIMap uses it
- Exclusion notices on provider pages, only where the LEIE record contains that provider’s NPI
Limitations
- Most LEIE records do not include an NPI. NPIMap never matches exclusions by name, so the absence of a notice does not mean a provider is not excluded.
- Always confirm exclusion status with the OIG online search before relying on it.
